PARENT-SCHOOL AGREEMENT CERTIFICATE

We have received and read the 2006-2007 Lebanon Catholic School Handbook.
The rules in the Handbook (and any addenda during the school year) are the governing
principles of Lebanon Catholic School.

We agree to abide by the Handbook, including dress code, tuition, profit sharing
policy, discipline, and all other sections.
