A gift book can be a meaningful memorial
fora friend, a way to share their memory with our school.

1. Complete this form on the flap of this folder.
2. A $15 to $25 donation places a book in the collection with a commemorative nameplate.  A larger donation may be indicated by checking “more” and will be put towards reference books.
3. Send or bring the completed form and your contribution to the library.
4. The librarian selects the book, and a nameplate is placed in the book according to the donor’s wishes.
5. The Library sends a letter acknowledging the gift to the person (s) or organization indicated by the donor.
