Dear Parent:

The Pennsylvania Department of Health, with support from the Pennsylvania Department of Education,

requires all children entering school for the first time in pre-K, kindergarten or

the first grade to have the following immunizations:

3 or more doses of oral polio vaccine

4 doses of tetanus, diphtheria toxoid

(5^th dose is required if the 4^th was given before the 4^th birthday)

2 doses MMR (measles, mumps, rubella) vaccine (1^st dose given on or after 1^st birthday)

3 doses hepatitis B vaccine (#1 & #2 must be 28 days apart.

#1 & #3 must be at least 6 months apart if #1 was given at birth.)

Proof of varicella vaccination or proof of disease

Grades 2nd thru 6^tfi need all of the above except varicella

7th, 8th, and9^th grades need records of everything listed under Pre-k, Kindergarten & 1^st grade.

10^th, 11^th, and 12^th grades need:

Polio 3

DPT-3

MMR -2

Mandatory exams by PA State Law

Dental - for incoming (pre-K, Kindergarten, 3^rd and '7^th

Physical --for incoming (pre-K, Kindergarten, 6^ith and 11^th
